Class Disclosures and Policies
Disclosure Statements:
Faculty and planners are void of any influencing financial relationships. If the situation ever arises, the participants will be informed prior to the start of the program.
There is no implied endorsement by Commonwealth Health Corporation, The Medical Center or American Nurses Credentialing Center of any product ever mentioned in conjunction with an activity.
In the event there is ever commercial support or activities related to the off-label usage of FDA approved products, then the learners will be informed of these situations as well.
Registration:
Registration is required for all offerings to ensure adequacy of space and material. Registration must be received in the department at least 48 hours in advance. Late registrations will be accepted if space is available.

Attendance:
All participants are required to provide their current professional license number, Social Security Number and picture ID when attending any offering. To receive contact hours, participants must attend the entire offering and participate in any pre- or post-testing when applicable. Partial credit will not be given.
Cancellation and Refund:
Registration may be cancelled. Registration fee will be returned minus $50 (or 20%) processing fee. Please send an email message to EDU@chc.net or call Education and Development at 270-796-2566.
Changes to Courses or Cancellations:
When there are cancellations or changes to courses, every effort will be made to notify participants via email and telephone.
